Standard #1 of the Medicare DMEPOS Supplier Standards states 'if a State requires licensure to furnish certain items or services a DMEPOS supplier must be licensed to provide the item or service'.

This guide contains the licensure requirements for each state as it pertains to the National Supplier Clearinghouse.

Helpful Tips:

  • When looking at the database, if you see "Y" in the column, that means "yes". If the "Y" is located in the Verify Online column, you can click the link to go to the website to verify information.
  • The Required for Pharmacies column will state whether or not a certain license is required for licensed pharmacies. If there is no "Y" present, that specific license is not required for licensed pharmacies.
  • If "See Notes" appears in the Notes column on the database, it is imperative you read the note associated with that license. There may be specific exemptions or other important information regarding the license noted.
  • This database is product and service driven and not supplier specialty driven.  The accreditation codes are also noted with the products/services for your convenience.

Note: In the case of home based businesses, the supplier is responsible for ensuring they are compliant with local ordinances regarding a home based business.
